Your Territory: Western Metro & Buffalo Area

We're hiring a PTA to serve patients throughout the Western Twin Cities Metro and communities extending toward Buffalo and surrounding areas.

Your territory may include:

Cokato, Corcoran, Delano, Greenfield, Hamel, Howard Lake, Independence, Long Lake, Loretto, Maple Plain, Medina, Montrose, Mound, Rockford, Waverly, Watertown, Hanover, Annandale, Kimball, Maple Lake, South Haven, Buffalo, and Cold Spring.

Scheduling is coordinated geographically whenever possible to help cluster patient visits and create a more efficient field schedule.

What You'll Do

Working under the direction of the supervising Physical Therapist, you'll:

• Provide one-on-one Physical Therapy treatments in patients' homes
  
• Carry out individualized plans of care established by the Physical Therapist
  
• Provide therapeutic exercise and strengthening
  
• Perform gait, balance, transfer, and mobility training
  
• Help improve endurance, coordination, and functional independence
  
• Reinforce home exercise programs
  
• Educate patients and caregivers on safe mobility and therapeutic techniques
  
• Monitor patient response to treatment
  
• Recognize and communicate changes in condition
  
• Provide progress updates to the supervising Physical Therapist
  
• Collaborate with nursing, OT, SLP, physicians, and interdisciplinary clinicians
  
• Complete timely and accurate clinical documentation

• Ability to recognize and communicate changes in patient condition

• Valid driver's license and reliable transportation
