ABOUT OREGON INTEGRATED HEALTH

Oregon Integrated Health is a team of clinical providers focusing on Integrative Primary Care.  We have the intention and commitment to provide an innovative system of medical care for patients' healthcare needs. We are a patient-centered primary care home with a large patient demographic of Oregon Health Plan patients. 

We are a Tier 4 Patient Centered Primary Care Medical Home that integrates mental/behavioral health services with primary care for all our patients. 

OIH has locations in Eugene, Florence, Portland and Salem.  Positions are available in Florence and there are options to work in our clinic in Eugene as well.

ABOUT THE PHYSICAL THERAPY POSITION AT OIH

The majority of the patient population are Medicaid/Medicare patients.

All administrative tasks such as; scheduling, checking in patients, insurance eligibility, prior authorizations, record requests and contracting/credentialing is managed by Oregon Integrated Health.

Physical Therapy sets their own schedule - Days/Times per week. Currently the clinic in Florence is open 800am - 700pm M-TH.   Position is flexible part time.
