DUTIES OF THE POSITIONPhysical therapists provide services that help restore function, improve mobility, relieve pain
and prevent permanent physical disabilities of patients suffering from injuries or disease.
Restore, maintain, and promote overall fitness and health. Their patients include accident
victims and individuals with disabling conditions such as low back pain, arthritis, heart disease,
fractures, head injuries, stroke, and cerebral palsy.
KEY RESPONSIBILITIES- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Providing initial and ongoing assessment to determine level of functioning, including OASIS Assessment at appropriate time points.
- Developing and revising the plan of care in consultation with the physician and other care-team members.
- Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Responsible for planning, evaluation, and treatment implementation of assigned caseload.
- Preparing clinical progress notes.
- Supervising therapy assistants and home health aides as appropriate.
- Evaluating outcomes.
- Educate patients and/or families about proper safe use and recommend methods for installation of adaptive equipment.
- Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
- Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
- Provide oversight and supervision to supportive staff including home health aides and physical therapy assistants.
- Participate, as requested, in agency evaluation activities as part of the Continuous Quality Improvement process.
- Other duties as assigned.
JOB CONDITIONS- Position is stressful in terms of meeting deadlines.
- Position is primarily a field role which essentially involves driving, standing, stooping, and walking, as well as an inordinate amount of documentation and telephone communication.
- The work requires moderately heavy physical exertion on a regular and recurring basis such as: extensive driving, assisting patient in transfer activities (wheelchair, to bed, to tub, to commode) and providing substantial support to individuals in ambulation therapy.
- The ability to utilize a PC, calculator, telephone, and other related office equipment.
- Travel is required, by car or airplane to patient homes, facilities, local, or state seminars, conferences, or meetings.
- The ability to communicate well, both verbally and in writing is required.
POSITION PREREQUISITES AND QUALIFICATIONS- A current Pennsylvania Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
