Physical Therapist/ Home Care Case Manager\n\n South Dayton, Miamisburg, Germantown, Middletown, Dayton,Englewood, Vandalia, Springfield, Urbana, New Carlisle and Yellowsprings\n\n \n\nGeneral Summary/Responsibilities: \nThe Physical Therapist evaluates and then directly provides or supervises the provision of physical therapy services to patients in their homes or residences. When functioning in the capacity of the Home Care Case Manager (HCCM), the Physical Therapist is responsible for the provision and coordination of all components of a patient\u2019s home care services. The Physical Therapist provides care as ordered by the physician, follow policies, procedures, and conditions of participation, and regularly reports to their supervisor. The incumbent facilitates the development and implementation of the plan of care as outlined by the physician. In addition, the incumbent is responsible for all required documentation related to the care they provide and communicates pertinent information regarding the patient to all appropriate health care team members.\n\nFIDRNOCT \n \n\nQualifications\n\nEducation\n\nMinimum Level of Education Required: Bachelor\u0027s degree \n \nAdditional requirements: \n\uf0a7 Type of degree: Bachelors - Graduate of an accredited Physical Therapy Program. \n\uf0a7 Area of study or major: Physical Therapy \n \n\nLicensure/Certification/Registration\n\n\uf0a7 Current licensure to practice Physical Therapy in Ohio. \n\uf0a7 CPR Certification\n\nExperience\n\nMinimum Level of Experience Required: 1 - 3 years of job-related experience \nPrior job title or occupational experience: Clinical experience should have occurred within the last three years unless currently employed in home care. \nPrior specific functional responsibilities: N/A \nPreferred experience: One year clinical experience in acute care setting preferred \nOther experience requirements: N/A\n\nKnowledge/Skills\n\n\uf0a7 Must be capable of repeated standing, stooping, bending and lifting up to 50 lbs. \n\uf0a7 Must be able to creatively adapt multiple environments and situations for successful outcomes. \n\uf0a7 Must be self-directed and able to work with little supervision. \n\uf0a7 Computer skills preferred. \n\uf0a7 Home care experience preferred.\n\n \n
