Physical Therapist Home Health

We are seeking a skilled and compassionate Physical Therapist (PT) to join our Home Health team in Harrisburg, PA. This is a full-time opportunity offering competitive pay ($72$80/hr) and the chance to provide meaningful, one-on-one care to patients in the comfort of their homes.

Key Responsibilities

• Perform thorough physical assessments and review medical histories to evaluate patient conditions.

• Develop and implement individualized treatment plans tailored to patient rehabilitation goals.

• Administer therapeutic exercises and mobility training programs to restore function.

• Educate patients and families on injury prevention, wellness strategies, and recovery techniques.

• Monitor, document, and adjust treatment progress toward rehabilitation objectives.

• Collaborate with nurses, physicians, and other healthcare professionals to ensure coordinated care.

• Maintain accurate documentation in compliance with regulatory and agency standards.

Qualifications

• Masters degree in Physical Therapy (Doctorate preferred).

• Minimum 1 year of home health physical therapy experience.

• Current state PT license (Pennsylvania).

• CPR certification (must be current).

• Valid drivers license and reliable transportation.

• Strong interpersonal and communication skills with a patient-first mindset.
