Position Overview

As a Home Health Physical Therapist, you'll provide vital rehabilitation services to patients in their homes throughout Franklin County, NY. This role combines clinical excellence with the flexibility of managing your own schedule while making a meaningful impact in patients' lives.

Essential Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans
• Deliver evidence-based physical therapy interventions in home settings
• Document patient progress and maintain detailed electronic health records
• Collaborate with interdisciplinary healthcare teams
• Educate patients and caregivers on rehabilitation techniques and equipment use
• Participate in care planning and case conferences

Qualifications

• NYS Physical Therapy license in good standing
• Graduate of an accredited Physical Therapy program
• Minimum 1 year of licensed PT experience
• Strong interpersonal and documentation skills
• Valid driver's license and reliable transportation
