Position Overview

We're seeking an experienced Physical Therapist to provide home-based therapeutic services throughout the Tupper Lake and Malone areas. This role combines clinical excellence with the flexibility of home health care delivery.

Key Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans
• Deliver evidence-based therapeutic interventions in patients' homes
• Document patient progress and maintain detailed clinical records
• Collaborate with healthcare team members to ensure optimal patient outcomes
• Educate patients and caregivers on therapeutic exercises and equipment use
• Participate in care planning and case conferences

Required Qualifications

• Current New York State Physical Therapy license
• Graduate degree from an accredited Physical Therapy program
• Minimum one year clinical experience
• Valid driver's license and reliable transportation
• Strong communication and interpersonal skills
