Physical Therapist I - AVN, Full Time, Sussex Co

Atlantic Health Atlantic Visiting Nurse is searching for a full time Physical Therapist for the Morris County territory. Full-time Team Members work 37.5 hours per week and this position will require the candidate to work one weekend every five to seven weeks. The ideal candidate will have at least one year of experience and able to work independently. The candidate will be responsible for providing a broad range of physical therapy services in an interdisciplinary setting, including assessment, treatment planning, and therapeutic interventions in accordance with the position's qualifications, professional practices, and ethical standards. The Physical Therapist will ensure and provide development, quality improvement, problem-solving, and productivity enhancement in a flexible interdisciplinary manner.

Atlantic Visiting Nurse provides personalized care for you or a loved one to regain health, stay out of the hospital, and remain living independently at home. Whether or not it is managing life with a chronic illness, living with a disability, returning home from the hospital or rehabilitation facility, or need assistance with end-of-life care, we are available to you in six counties throughout New Jersey: Morris, Somerset, Essex, Sussex, Union, and Warren. 

Principal Accountabilities:


1. Performs initial examinations and differential diagnosis.
2. Provides and prescribes plans of care consisting of corrective therapeutic exercise.
3. Performs manual treatments consisting of soft tissue techniques, joint mobilizations, and many more.
4. Supervises and interacts with support staff and maintain proper documentation.
5. Evaluates and establishes a plan of care and treats patients accordingly.
6. Documents patient care and charges appropriately, and documents the course of patient care, including progress made and the ongoing need for treatment.
7. Works with other professionals in an effective manner
8. Provides clinically appropriate therapeutic interventions.
9. Evaluates patient response to treatment and provides feedback to the interdisciplinary team.
10. Performs other related duties as assigned.

Essential Functions/Responsibilities:

• Evaluate and treat patients in their homes

Minimum Education:

• High School Diploma or GED, required
• Bachelor's in Physical Therapy from an accredited college/university, required
• Master or Doctorate in Physical Therapy preferred

Minimum Experience:

• 1 year of experience in homecare, strongly preferred

License/Certification:

• NJ License in Physical Therapy
• BLS required 
• Valid and unrestricted Driver License with a clean driving record, required.
