Job Description - Physical Therapist II - Home Health

Bargaining Unit: EAP


Rate of Pay: $57.59/hour + DOE


Summary
Under the direct supervision of the clinical manager and director, the Home Health Physical Therapist (PT) II is responsible for providing patient care services consisting of assessments, evaluation and implementation of modalities and treatments, and develop appropriate treatment plans directly to patients within the home setting in accordance with established departmental policies and procedures and as indicated by licensing agencies in the State of practice (CA/NV). The Home Health PT II is committed to providing high quality health care services and to accommodating to the changing needs of the patient in the home setting. The Home Health PT II performs additional duties for the Home Health/Hospice department related to facilitating therapy schedule and performing and reporting Quality Assurance (QA) audits as requested by management staff.


Essential Duties and Responsibilities



  • Organizes and conducts medically prescribed Physical Therapy programs within the home setting.

  • Effectively utilizes appropriate resources to ensure sensitive, personal care to individuals who require physical therapy services.

  • Assess Home Health patients and provides appropriate treatment plans, and performs individualized therapy programs for each patient, which includes but is not limited to, soft tissue massage/mobilization, traction, therapeutic exercise, manual techniques, durable medical equipment training and safety assessments and recommendations in the home setting.

  • Collaborates with other home health team members to assure that services and activities can be properly maintained to meet the needs of the patients.

  • Applies diagnostic and prognostic muscle, joint and functional ability tests.

  • Evaluates, documents, and reports patient progress for review of the home health team.

  • Assures that patients, care takers, and families follow established policies and procedures at all times.

  • Interprets physical therapy policies and procedures to patients, care takers, and family members, as necessary.

  • Participates in peer feedback to improve performance of line of service.

  • Clinician will practice fiscal responsibility and strive to maintain minimum targeted key performance indicators as set by Director and Clinical Manager of department.

  • Extends courtesy and consideration to others in interpersonal relationships.

  • Demonstrates a professional manner in the workplace at all times.

  • Seeks clarification of duties when needed.

  • Channels suggestions, criticisms, and questions to appropriate personnel.

  • Facilitates and reports therapy audits related to the Quality Improvement process and Quality Assurance (QA) program in cooperation with home health management staff.

  • Participates in staff meetings or reviews meeting minutes if not present.

  • Working with home health management, facilitates therapy scheduling assuring required therapy visits are performed.

  • Follows standard operational workflows established for the department (includes preferred communication methods to other staff members, EPIC documentation requirements, and patient scheduling).

  • Establishes good rapport with all team members, patients and other health care providers.

  • Instructs, motivates and assists patients to perform physical activities which may require assistive devices in the home setting.

  • Instructs family, care takers and patients in patient education and home programs and assess compliance and performance in the home setting.

  • PT will participate in activities to meet the health needs of people locally, nationally, or globally as recommended by the American Physical Therapy Association (APTA) code of ethics.

  • Demonstrates System Values in performance and behavior.

  • Complies with System policies and procedures.

  • Additional duties may include:


    • In collaboration with leadership, therapist will help facilitate and coordinate meetings, record minutes, and reporting for assigned specialty group.

    • In collaboration with leadership, therapist will complete tasks for assigned project to support existing service lines of the department. (i.e.- Scheduling, Staff/Student Education and Training, Quality Assurance, Performance Improvement, Compliance, In-services)

    • In collaboration with leadership, therapist will assist in development of new service lines, strategic plan initiatives, and/or programs of the department

    • In collaboration with leadership, therapist will facilitate training and participate in mentoring, clinical education, and staff performance improvements

    • Perform Hospice assessments/interventions within scope of practice as ordered.

Minimum Education/Experience
Bachelor's Degree of Physical Therapy from an accredited physical therapy program, be licensed to practice and pass the National Physical Therapy Examination administered by the Federation of State Boards of Physical Therapy (FSBPT)


2-3 years relevant experience


Required Licenses/Certifications
Basic Life Support (BLS) for Healthcare Providers Upon hire


BLS: Employee will be enrolled in the Resuscitation Quality Improvement (RQI) Basic Life Support (BLS) Entry or Prep Curriculum (depending on their previous BLS certification). Within 3 months of hire into job


Other Experience/Qualifications


Required



  • California Physical Therapy license if practicing in California

  • Nevada Physical Therapy license if practicing in Nevada

  • Bachelors or Certificate degree from an accredited Physical Therapy Program


Preferred



  • Masters or Doctorate Degree from an accredited Physical Therapy Program
