Physical Therapist (PT) – LTC/SNF - Travel Position

Job Description:

We are seeking a compassionate Physical Therapist (PT) to join a skilled nursing and long-term care facility in Stamford, CT, on a travel contract. The PT will evaluate, plan, and deliver therapy services to residents, focusing on improving mobility, restoring function, and enhancing overall quality of life. The ideal candidate is patient-centered, detail-oriented, and experienced in geriatric rehabilitation.

Key Responsibilities:

• Conduct thorough patient evaluations to assess mobility, function, and therapy needs.


• Develop and implement individualized treatment plans in collaboration with physicians and care teams.


• Provide therapeutic interventions including strengthening, gait training, and balance activities.


• Educate residents, families, and caregivers on therapy programs, safe mobility, and the use of assistive equipment.


• Document evaluations, progress notes, and treatment outcomes in compliance with facility and regulatory standards.


• Collaborate with interdisciplinary teams to ensure coordinated care and optimal patient outcomes.


• Promote patient independence, safety, and quality of life through evidence-based therapy.

Qualifications:

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.


• Current and valid Physical Therapist license in Connecticut (or compact license).


• Minimum of 1 year of PT experience, preferably in long-term care or skilled nursing facilities.


• Current Basic Life Support (BLS) certification.


• COVID vaccine and recent TB test.


• Strong communication, assessment, and organizational skills.


• Compassionate, patient-focused approach with experience supporting geriatric populations.
