Physical Therapist – LTC/SNF - Travel Position

Pay: $2,035/week

Job Description:

We are seeking a dedicated and compassionate Physical Therapist to join a skilled nursing and long-term care facility in Snow Hill, NC, on a travel contract. The Physical Therapist will evaluate residents, design individualized treatment programs, and provide therapy services that promote mobility, restore function, and enhance independence. The ideal candidate is patient-focused, detail-oriented, and experienced in geriatric rehabilitation.

Key Responsibilities:

• Conduct comprehensive evaluations to determine mobility, strength, and therapy needs.


• Develop and implement individualized treatment plans in coordination with physicians and care teams.


• Deliver therapeutic interventions including strengthening, gait training, balance exercises, and mobility support.


• Educate residents, families, and caregivers on therapy goals, safe practices, and assistive equipment.


• Document evaluations, progress notes, and treatment outcomes accurately and timely.


• Collaborate with interdisciplinary staff to ensure coordinated, patient-centered care.


• Promote safety, independence, and quality of life through evidence-based therapy services.

Qualifications:

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.


• Current and valid Physical Therapist license in North Carolina (or compact license).


• Minimum of 1 year of recent PT experience, preferably in skilled nursing or long-term care.


• Current certification in Basic Life Support.


• COVID-19 vaccine and recent tuberculosis test.


• Strong communication, assessment, and organizational skills.


• Compassionate approach with experience supporting geriatric populations.
