Outpatient Physical Therapist or Physical Therapist Assistant (Full-Time) - WV Multiple Locations

H2 Health is hiring full-time Physical Therapists to join our growing network of outpatient clinics in the state of West Virginia. This opportunity is perfect for clinicians who are looking for clinical autonomy and a flexible schedule. Requirements: 

• Completed Physical Therapy or Physical Therapist Assistant degree from an accredited program 
• Active or eligible for state license (or eligibility to obtain) 

Job Responsibilities: 

• Provide evidence-based physical therapy to patients with post-op, orthopedic, and neurologic conditions to promote functional independence, reduce pain, and improve quality of life 
• Collaborate with other members of the multi-disciplinary team and support staff to deliver high-quality, coordinated care 
• Maintain accurate and timely documentation in Raintree
