Physical Therapist (PT) Outpatient Rehabilitation – Travel Position

Job Description:

We are seeking a dedicated Physical Therapist (PT) to join an outpatient rehabilitation facility in Cook, MN, on a travel contract. The PT will evaluate and treat patients recovering from injuries, surgeries, and chronic conditions to restore function, improve mobility, and enhance quality of life. The ideal candidate is self-motivated, patient-centered, and skilled in providing individualized outpatient therapy programs.

Key Responsibilities:

• Conduct patient evaluations to assess physical function, strength, mobility, and rehabilitation needs.


• Develop and implement personalized treatment plans in alignment with physician recommendations.


• Deliver therapeutic interventions including strengthening, balance, functional mobility, and pain management techniques.


• Educate patients and families on therapy goals, home exercise programs, and preventive care strategies.


• Track and document patient progress, treatment sessions, and outcomes in compliance with facility and regulatory standards.


• Collaborate with physicians, nurses, and interdisciplinary team members to ensure coordinated patient care.


• Maintain a safe and supportive outpatient treatment environment.

Qualifications:

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.


• Current and valid Physical Therapist license in Minnesota (or compact license).


• Minimum of 1 year of recent outpatient or orthopedic physical therapy experience.


• Current Basic Life Support (BLS) certification.


• COVID vaccine and recent TB test.


• Strong interpersonal, clinical, and organizational skills.


• Compassionate approach with ability to motivate patients through rehabilitation.
