Your Role
As a Physical Therapist at Remedy, you’ll deliver one-on-one care that helps patients regain independence and improve quality of life. You’ll develop customized treatment plans, focus on fall prevention, and educate patients and their families on best practices for recovery and mobility.

Key Responsibilities

• 
  

  Conduct in-home physical therapy evaluations and treatments

  
  


• 
  

  Create and implement individualized care plans

  
  


• 
  

  Help patients improve strength, balance, mobility, and daily functioning

  
  


• 
  

  Educate patients and caregivers on safety techniques and equipment use

  
  


• 
  

  Accurately document visits using the WellSky Kinnser EMR system

  
  


• 
  

  Communicate effectively with physicians, case managers, and the care team

  
  

Who You Are

• 
  

  A licensed Physical Therapist in the state of California

  
  


• 
  

  CPR certified through the American Heart Association

  
  


• 
  

  Experienced in home health or at least one year in clinical practice

  
  


• 
  

  Compassionate, reliable, and organized

  
  


• 
  

  Equipped with a valid driver’s license, car insurance, and dependable transportation

  
  


• 
  

  A strong communicator with excellent clinical judgment
