Responsibilities:

As a Physical Therapist working in an inpatient setting, you will be an integral part of the care team, supporting patients through recovery in a hospital or acute rehab environment:

• Evaluate and treat patients with post-surgical, neurological, orthopedic, and complex medical conditio
• Create and implement evidence-based treatment plans aimed at improving mobility, strength, and overall function.
• Collaborate with nurses, physicians, occupational therapists, speech-language pathologists, and discharge planners.
• Track patient outcomes and document progress accurately using EMR.
• Ensure continuity of care by assisting with safe discharge planning and home transition recommendations.
• Maintain a safe, clean, and supportive therapeutic environment that reflects H2 Health’s high standards of care.

Requirements:

• Education: Must have a degree from an accredited Physical Therapy program (DPT or equivalent).
• License: An active or eligible state license to practice as a Physical Therapist (PT) 
• Experience: Open to new graduates! Inpatient, acute, or rehab hospital experience is a plus, but strong clinical skills and a passion for patient care are what matter most.
