• Develop and implement individualized treatment plans using evidence-based practices
• Complete OASIS documentation and maintain detailed patient progress records
• Collaborate with an interdisciplinary healthcare team to ensure optimal patient outcomes
• Provide patient and caregiver education on rehabilitation techniques and injury prevention
• Supervise Physical Therapy Assistants and Home Health Aides
• Participate in quality improvement initiatives and outcome measurements

Required Qualifications:

• Current Physical Therapy license in state of practice
• Graduate degree from an APTA-accredited Physical Therapy program
• Minimum 2 years of healthcare experience (home health preferred)
• Valid driver's license and reliable transportation with proper insurance
• Current CPR certification
• Strong communication and interpersonal skills
• Proven ability to work independently and manage time effectively
