Senior Home Health Physical Therapist - Brenham, TX

We are seeking a dedicated Senior Physical Therapist to join our premier home health team in Brenham, Texas. In this role, you'll provide exceptional rehabilitation services to patients in their homes, making a meaningful impact on their recovery and quality of life. This full-time position offers a competitive annual salary range of $105,000 - $115,000.

Essential Responsibilities:

• Perform comprehensive physical therapy evaluations and develop evidence-based treatment plans
• Deliver hands-on therapeutic interventions and mobility training in patients' homes
• Create and implement individualized exercise programs focused on functional improvement
• Provide expert patient and caregiver education on rehabilitation techniques
• Document patient progress using electronic health records (EHR)
• Coordinate care with physicians, nurses, and other healthcare professionals
• Maintain compliance with Medicare and insurance requirements

Required Qualifications:

• Current Physical Therapy license in Texas
• Graduation from an accredited DPT program
• Minimum 9 months of clinical experience
• Prior home health experience required
• Valid driver's license and reliable transportation
• Strong clinical assessment and documentation skills
• Excellence in patient communication and education
