Physical Therapist (PT) - Home Health, Full -Time or Per Diem

There For You Home Care Services a Division of UniversaCare is hiring Physical Therapists for home health visits in the Waterbury, CT area. Full-time and per diem schedules are available: per diem clinicians take visits on a per-visit basis that fits their availability, and full-time therapists carry a steady caseload with hourly pay. We are a Medicare-certified home health agency (CT DPH License 9915730) and offer a supportive interdisciplinary team, manageable caseloads and clear documentation standards.

Responsibilities

• Complete initial and ongoing PT evaluations in the patient’s home, including OASIS data collection when PT is the assessing discipline


• Develop and update an individualized plan of care with measurable functional goals


• Provide skilled interventions: gait and balance training, strengthening, transfer training, post-surgical and orthopedic rehabilitation, fall-risk reduction


• Recommend and train patients on durable medical equipment and home modifications


• Educate patients, families and caregivers on home exercise programs and safety


• Coordinate with nursing, OT, SLP, aides and physicians; communicate changes in status


• Complete timely visit notes, progress notes, recertifications and discharge summaries that meet Medicare and agency standards

Requirements

• Current Connecticut Physical Therapist license (required)


• DPT or equivalent degree from an accredited program


• Home health experience and OASIS familiarity preferred; new home health clinicians with strong clinical skills are welcome


• Valid driver’s license, reliable vehicle and auto insurance


• CPR certification

Compensation

• Full-time: $48.00 - $58.00 per hour


• Per diem: $90.00 - $110.00 per visit

Work location: in patients’ homes, based from our Waterbury office (534 Frost Road) and serving Greater Waterbury, the Naugatuck Valley and surrounding New Haven and Litchfield County towns. Hamden-area visits also available.
