Physical Therapist PT Home Health (PRN Weekends)

Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health Physical Therapist (PT) to work with us to provide exceptional and individualized care to patients in Sarasota county.

Locations: Home health visits available throughout Sarasota county

Schedule: PRN Weekend role. Must be available on weekends (ideally both Saturday & Sunday) for this particular role

What you'll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals


• Develop, initiate, and revise individualized plans of care as patient needs evolve


• Deliver skilled occupational therapy services and treatments as outlined in the plan of care


• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies


• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

• Active Florida PT license


• Minimum of 1 year experience as a licensed Physical Therapist (PT)


• Home health care experience strongly preferred.


• Strong assessment, critical thinking, and patient education skills


• Willingness to participate in weekend and holiday on-call rotations
