Core Responsibilities:

• Conduct comprehensive patient evaluations and develop personalized treatment plans
• Deliver evidence-based therapeutic interventions including manual therapy, gait training, and therapeutic exercise
• Perform detailed home safety assessments and recommend adaptive equipment
• Provide expert patient/caregiver education on exercises, fall prevention, and safe mobility
• Maintain accurate documentation meeting Medicare/Medicaid requirements
• Collaborate with interdisciplinary healthcare teams
• Manage a daily caseload of 5-7 patients within assigned territory

Required Qualifications:

• Master's degree in Physical Therapy required
• Current state Physical Therapy license
• Minimum 1 year home health experience
• Valid driver's license and reliable transportation
• Current CPR certification
• Strong communication and interpersonal skills
• Ability to work independently and manage time effectively
