Job Description - Physical Therapist (PT) - Home Healthcare - Continuous Learning Opportunities

We are hiring a driven Physical Therapist (PT) - Home Healthcare to join our incredible team at SAFREST in Philadelphia, PA.

We are seeking a highly motivated and skilled Physical Therapist. The Physical Therapist will play a crucial role in helping patients regain mobility, improve their quality of life, and achieve their rehabilitation goals.

This is a contract opportunity in home healthcare, part-time or full-time (20 - 40 hours per week).

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Conduct thorough assessments of patients' physical conditions, including strength, range of motion, coordination, and balance.
  • Develop and implement individualized treatment plans based on patients' needs and goals.
  • Provide hands-on therapeutic interventions, including exercises, manual therapy, and specialized techniques.
  • Educate patients and their families about the treatment process and home exercise programs.
  • Monitor patients' progress, adjust treatment plans as necessary, and document accurately and timely.
  • Collaborate with other healthcare professionals, such as physicians, nurses, and occupational therapists, to ensure comprehensive patient care.
  • Maintain a safe and clean working environment, adhering to all infection control and safety protocols.

QUALIFICATIONS:

  • Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
  • Valid state licensure or eligibility to obtain licensure as a Physical Therapist.
  • Experience in home healthcare as a Physical Therapist.
  • Strong knowledge of anatomy, physiology, and physical therapy principles and techniques.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients and collaborate with a multidisciplinary team.
  • Demonstrated ability to work independently, prioritize tasks, and manage time effectively.
  • CPR certification (BLS or equivalent).
  • Home health care experience and experience with OASIS is required.
  • Experience with Homecare Homebase is a plus!
