Daily Duties:

• Provide therapy sessions in home environments
  
• Develop and execute individualized care plans
  
• Participate in care team meetings
  
• Maintain electronic records and compliance
  

What You Need:

• MN PT license in good standing
  
• Strong interpersonal and assessment skills
  
• Ability to work independently in the field
