Job Description - Physical Therapist / PTA - Traverse City (Full Time or PRN)
We are seeking a Home Health Physical Therapist and Physical Therapist Assistants to join our team at AdvisaCare! In this role, you will work directly with clients to evaluate, develop, and implement individualized treatment plans that improve mobility, restore function, and enhance quality of life. Physical Therapists (PTs) will complete comprehensive assessments, establish goals of care, and oversee therapy services. Physical Therapist Assistants (PTAs) will provide hands-on treatment under the supervision of a licensed PT, carrying out therapeutic exercises and interventions in alignment with the plan of care.
You will educate patients and families, promote safe and effective rehabilitation, and collaborate with interdisciplinary healthcare teams to ensure coordinated, patient-centered care.

Responsibilities
Assist in the implementation of individualized treatment plans developed by the licensed Physical Therapist.
Conduct therapeutic exercises and activities with clients, focusing on improving mobility, strength, and coordination.
Provide guidance and education to clients and their families on proper techniques for exercises, transfers, and ambulation.
Assess and document clients' progress, making necessary adjustments to treatment plans as needed.
Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal patient outcomes.
Communicate effectively with clients, families, and caregivers, providing support and answering questions.
Adhere to all relevant legal, ethical, and professional standards.
Degree from an accredited Physical Therapy Assistant program.
Current licensure as a Physical Therapist Assistant in the state of Michigan.
CPR certification.
Strong interpersonal and communication skills.
Ability to work independently and as part of a team.
Flexibility to adapt to changing client needs and schedules.
