What You'll Do

✅ Evaluate patients and develop individualized treatment plans

✅ Provide skilled physical therapy services in the home setting

✅ Collaborate with physicians, nurses, and interdisciplinary team members

✅ Educate patients and caregivers on treatment plans and home exercise programs

✅ Complete timely and accurate clinical documentation

Qualifications

✔️ Current Oregon Physical Therapist License

✔️ Valid Driver's License and reliable transportation

✔️ Home Health experience preferred, but not required

✔️ Strong communication, assessment, and clinical skills
