About the Role:

As a Full-Time Physical Therapist, you'll play a vital role in assessing, treating, and guiding residents through personalized rehabilitation plans. You'll be part of an interdisciplinary team committed to delivering exceptional care within a skilled nursing and long-term care setting.

Key Responsibilities:

• Perform comprehensive evaluations and develop individualized treatment plans.

• Use evidence-based physical therapy techniques to improve residents mobility, strength, and function.

• Provide pain management through manual therapy, therapeutic exercise, and adaptive equipment.

• Educate residents and families on therapy objectives, outcomes, and recovery expectations.

• Adjust care plans based on clinical assessments and patient progress.

• Supervise and mentor physical therapy assistants when applicable.

• Maintain accurate, timely documentation in compliance with state and facility regulations.

• Collaborate with nursing staff, occupational therapists, and other care providers to ensure holistic, patient-centered treatment.

Qualifications:

• Degree from an accredited Physical Therapy program.

• Current New York State Physical Therapist (PT) license in good standing.

• Experience in a skilled nursing facility (SNF) or long-term care setting preferred.

• Strong interpersonal skills with a compassionate and patient-centered approach.

• Ability to work effectively within a multidisciplinary healthcare team.
