Physical Therapist – Skilled Nursing Facility – New York

Salary Range: $90,000 – $110,000 per year

Location: Syracuse, New York

Job Type: Full-Time

Schedule: Monday to Friday, 9 AM – 5 PM

Are you a Physical Therapist looking to make a meaningful impact in the lives of seniors? Our skilled nursing facility in New York is seeking a licensed PT to join our care team and help improve the mobility, function, and quality of life for our residents.

Key Responsibilities:

• Evaluate residents and develop individualized care plans
• Implement physical therapy treatment programs to restore function and reduce pain
• Work collaboratively with nursing and rehab staff to support holistic care
• Maintain accurate documentation in accordance with regulatory standards
• Educate residents, families, and caregivers on therapy exercises and goals
• Monitor and report progress to physicians and care team members

• Active New York Physical Therapist license (required)
• Graduate of an accredited Physical Therapy program
• Experience in a skilled nursing or long-term care setting preferred
• Strong interpersonal, communication, and organizational skills
• Empathy and passion for working with geriatric populations
