Your Role as a PT 

As a valued member of our team, you will: 

• Perform personalized in-home therapy sessions, developing tailored treatment plans that deliver real results. 

• Guide patients in improving strength, balance, and mobility, reducing fall risks, and enhancing independence. 

• Educate patients and families on safe daily living activities, such as using assistive devices and preventing falls. 

• Document treatment progress, patient status, and outcomes using the WellSky/Kinnser EMR system. 

• Collaborate with physicians and case managers to deliver seamless care. 

Who We’re Looking For 

We’re seeking passionate, self-motivated professionals who: 

• Hold a valid California PT license. 

• Have AHA CPR certification. 

• Are reliable, organized, and personable. 

• Have a minimum of one year of experience (Home Health experience is a plus). 

• Have reliable transportation, a valid driver’s license, and auto insurance. 

• Possess excellent communication and problem-solving skills.
