Always Best Care is a growing Medicare-certified Home Health agency dedicated to serving patients across O‘ahu. Our mission centers on empowering seniors to remain safe, independent, and thriving in the comfort of their own homes through targeted fall prevention, mobility enhancement, and strength building.

We are seeking a licensed Physical Therapist (PT) who is passionate about delivering high-quality, patient-centered care directly to local families and kupuna.

Responsibilities: 

• Delivering skilled, compassionate in-home physical therapy services.


• Conducting comprehensive initial evaluations, ongoing reassessments, and detailed fall risk assessments.


• Designing and executing individualized treatment plans focused on balance, strength, and spatial safety.


• Empowering patients and their multi-generational caregivers through tailored home exercise program education.


• Ensuring accurate, compliant clinical documentation and collaborating smoothly with our interdisciplinary care team.
  
  

What We Are Looking For:

• Active, Hawaii license as a Physical Therapist (PT).


• Minimum of one year of clinical experience (home health experience is a huge plus, but we welcome strong clinical backgrounds).


•  Excellent communication, time management, and independent clinical documentation skills.


• Valid driver’s license and reliable transportation to travel to patient homes.


• A genuine dedication to serving seniors and giving back to our local Oʻahu community.
