Key Responsibilities:
- Assist in implementing individualized physical therapy treatment plans under the direction of a licensed Physical Therapist (PT).
- Provide direct patient care in their homes, focusing on improving mobility, strength, and overall physical function.
- Educate patients and families on therapeutic exercises, home safety, and injury prevention techniques.
- Monitor patient progress and report any changes or concerns to the supervising Physical Therapist.
- Document patient progress and treatment interventions accurately and in a timely manner.
- Collaborate with the multidisciplinary team, including physicians, nurses, and case managers, to ensure coordinated patient care.
- Maintain compliance with all regulatory and safety standards for home health care.
Qualifications:
- Current and valid Physical Therapist Assistant (PTA) license in Virginia.
- Minimum of 1-2 years of experience as a PTA, preferably in home health care.
- Knowledge of physical therapy techniques, home health procedures, and patient care strategies.
- Strong communication and interpersonal skills.
- Ability to work independently while collaborating with a healthcare team.
- Compassionate, patient-centered approach to care.
- Familiarity with electronic health records (EHR) systems is a plus.
