Physical Therapist Assistant (PTA) – Home Health

We are currently seeking a compassionate and motivated Physical Therapist Assistant (PTA) who is passionate about delivering exceptional patient care in a home health setting. Key Responsibilities

• Deliver Physical Therapy treatments according to the plan of care developed by the Physical Therapist


• Collaborate with the interdisciplinary team to identify and assist with obtaining patient‑specific DME (mobility aids, wheelchairs, cushions, orthotics, adaptive equipment, etc.)


• Maintain accurate daily treatment records and progress notes as directed by the Physical Therapist


• Work with the Physical Therapist and clinical team to educate patients and caregivers on safe discharge planning

Requirements

• Graduate of an accredited Physical Therapist Assistant program


• Current Wisconsin PTA license/certification


• Current American Heart Association CPR certification
