Physician Assistant - Telehealth Position

Position Overview:

•    Provides medical care under direction of medical director and/or staff physician 

•    Conducts patient assessments via telemedicine in office based setting

•    Supports Montefiore Medical Group provider in baskets

Key Responsibilities:

•    Direct patient care in telemedicine model

•    Patient message review and remote triage 

•    Prioritizes care according to clinical urgency

•    Documentation of all patient related work in Epic EMR

•    Orders and interprets diagnostic procedures and lab data within scope of privileges

•    Monitors patients' medical conditions throughout treatment

•    Determines appropriate level of care (self-care, in-person or video visit, immediate attention)

Telehealth Duties:

•    Conducts telemedicine visits for chronic primary care and acute needs

•    Makes treatment decisions within scope of privileges 

Care Domains:

•    Preventive care

•    Chronic disease management

•    Acute care

•    Behavioral health care in primary care setting

Requirements:

•    3 years of post-training practice expereince as a Physician Assistant

•    Graduate of an accredited Physician Assistant program

•    Registered/Licensed as a Physician Assistant in New York State

•    DEA License

•    NCCP Certified
