Job Description - Physician Assistant (PA) Integrated Care | Outpatient | Bridgeport Area, Connecticut

Healthcare United is seeking an experienced Physician Assistant (PA) for an Integrated Care, 100% outpatient role in the Bridgeport area of Connecticut. This opportunity offers a collaborative, team -based setting focused on delivering high -quality, patient -centered care.

• Provide primary and integrated care services in an outpatient setting

• Conduct patient evaluations, physical exams, and clinical assessments

• Order and interpret labs, imaging, and diagnostic testing

• Develop and manage treatment plans in collaboration with supervising physicians

• Manage patient panels within a multidisciplinary care model

• Participate in team -based care coordination and case reviews

• One evening per week and rotating Saturday coverage (9am–1pm)

• Participate in telephonic call rotation

Requirements

• Licensed Physician Assistant (PA) in Connecticut

• Minimum 2 years of experience required (3+ preferred) in primary care, family medicine, or integrated care

• Strong clinical and patient management skills

• Bilingual Spanish preferred

• Commitment to team -based, community -focused care
