Physician - Hospitalist - for Skilled Nursing Facilities

Position: Full-Time OR Part Time Hospitalist to round at Skilled Nursing Facilities

Job Type: Full-Time Schedule: Monday to Friday (No Weekends) OR Part-Time (No Weekends), Flexible Schedule

Location: San Diego Area

Our mission is to provide high-quality and compassionate care for seniors in the comfort of their homes or senior living communities. As a primary care provider with Aligned Geriatrics you will play a crucial role in improving the lives of older adults by delivering personalized healthcare services.

Physician Responsibilities:

• Conduct regular rounding at the SNF, managing both acute and chronic conditions.
• Complete history and physicals on new admissions and oversee care transitions.
• 
  

  Respond to urgent medical issues during facility hours.

• 
  

  Collaborate with nursing, therapy, and administrative staff to ensure coordinated care.

• 
  

  Communicate with families and responsible parties regarding treatment plans and changes.

• 
  

  Document encounters and medical decision-making clearly and timely in the EMR.

• 
  

  Participate in quality assurance and performance improvement activities.

• 
  

  Follow all federal, state, and facility-specific regulations and protocols.

Qualifications:

• Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
• Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatric Medicine (preferred)
• Active and unrestricted California medical license
• Experience in Geriatric Medicine is a plus
• Strong communication and interpersonal skills
• Experience with Epic (EHR) is a plus
• Experience with Microsoft Office is a plus
• Strong time management and organizational skills
• Comfortable working independently and in team based environments
• Compassionate bedside manner with a commitment to providing patient centered care.
• Experience in skilled nursing or post-acute care preferred.
