Finger Lakes Community Health (FLCH) was founded in 1989 with an original mission of serving the region's agricultural workers by linking individuals to area resources and providing essential health services. In 2009 the organization expanded to become a Federally Qualified Health Center (FQHC), serving patients of all incomes, ethnicities and walks of life. Over the last decade, FLCH has grown to provide medical, dental, and behavioral health services to over 28,000 patients at eight health centers. Job Summary:


The Population Health Management Specialist is responsible for developing and maintaining data collection and reporting tools to analyze population health information. The role involves tracking patient outcomes, identifying care gaps, and creating strategies to improve health outcomes. The specialist collaborates with vendors, payers, and stakeholders to optimize contracts and foster teamwork while also identifying workflow improvements through data analysis. This role involves proactive outreach to patients and healthcare providers, leveraging data from various health information systems to address care gaps, and facilitating educational initiatives. Additionally, they manage administrative access to data platforms and build strong relationships to drive healthcare improvements through validated data.



Main Responsibilities:


Data Collection and Management



  • Create and maintain databases to track patient outcomes, utilizing platforms, EMRs, and payer information.

  • Utilize reports and registries to monitor patient quality metrics, address care gaps, and develop improvement strategies.

  • Access health information from internal and external repositories (e.g., Health e Connect, Rochester RHIO, NYSIIS) to ensure accurate and complete patient records.

  • Accurately upload and organize patient information in clinical records for effective reporting and analysis.

  • Conduct outreach to patients and healthcare providers to secure medical records, releases, and coordinate care.


Collaboration and Relationship Management



  • Foster relationships with vendors, payers, and stakeholders to optimize contracts and achieve goals.

  • Collaborate with teams to support coordination of best practices for patient care.


Workflow Optimization



  • Analyze scorecards, dashboards, and reports to identify trends and opportunities to improve patient compliance and healthcare outcomes.

  • Develop and implement new workflows based on data-driven insights to enhance care delivery.


Administrative Oversight of Data Platforms



  • Maintain and manage access to multiple data platforms, including setting up and shutting off staff access and resetting passwords.

  • Continuously monitor reporting systems to ensure data accuracy.


Communication and Stakeholder Engagement



  • Build strong, collaborative relationships with providers and stakeholders to drive change and improve efficiency using validated data.

  • Communicate effectively to secure necessary support for implementing improvements.



Education and Qualifications: 



  • High School or Equivalency Diploma

  • Prior experience in related field a plus

  • Possesses New York State driver’s license

  • The ability to communicate fluently in both Spanish and English is preferred



Physical Requirements: 



  • Must be able to hear and communicate with clients and staff via phone, web media, and in person.

  • Must be able to lift up to 30 lbs

  • Must be able to sit for extended periods of time

  • OSHA Level II
