Population Health Medical Assistant

SHHC is seeking an experienced Medical Assistant to join our Population Health team. ​ The MA will play a key role in supporting quality initiatives, patient outreach, care coordination, and clinical operations. ​ Responsibilities include scheduling appointments, pre-visit planning, participating in huddles, managing patient care gaps, and collaborating with the care team to ensure high-quality services. ​ This position reports to both Squirrel Hill and Brentwood office locations.

Key Responsibilities:

• Schedule follow-up appointments, immunizations, and in-house testing. ​
• Conduct pre-visit planning and identify gaps in care. ​
• Participate in daily huddles and share key patient data. ​
• Outreach patients for preventive screenings and track progress. ​
• Manage inbox tasks, relay lab results, and address patient inquiries. ​
• Collaborate with providers, RNs, and other SHHC teams to support care plans. ​

Qualifications: ​

• 3-5 years of experience as a Medical Assistant (certification preferred). ​
• Knowledge of medical terminology, ICD-10, and CPT coding. ​
• Proficiency in Electronic Health Records (EHR). ​
• CPR certification required. ​
• Bilingual skills preferred. ​
