Population Health Supervisor

Tanner Clinic has an immediate opening for a Population Health Supervisor.

Location: Layton, UT

Hours: Fulltime

Essential Job Responsibilities:

Assist the Population Health Manager in coordinating clinic efforts to meet requirements for federal and insurance-driven Population Health incentive programs. Support the execution of activities related to incentive programs, including data tracking, outreach, and reporting.

Work closely with Physicians, Population Health staff, and clinic personnel to ensure appropriate patient coordination, documentation, and follow-up. Assist in supervising Population Health staff in their efforts to:

· Engage with physicians and staff to facilitate completion of incentive-required care

· Monitor and organize data for incentive programs

· Maintain program timelines and meet reporting deadlines

· Conduct outreach to patients to schedule annual wellness visits and other preventative care services

Education: 

· Associate’s or Bachelor’s degree in a health-related or management field preferred.

Other Requirements:

· Regular and reliable in-office attendance is an essential function of the job.

Performance Requirements:

Knowledge:

· Familiarity with healthcare settings and understanding of Population Health concepts preferred.

Skills:

· Proficient with Microsoft Office. Comprehensive understanding of Microsoft Excel and its various functions preferred. 

· Proficient in the use of electronic medical records and related clinical systems. Strong organizational and communication skills.

Abilities: 

· Ability to manage and track data using spreadsheets and databases. Ability to support and coordinate efforts among staff and effectively communicate with clinical teams.

Equipment Operated: 

· Standard office equipment including computers, fax machines, copiers, printers, telephones, etc.

Work Environment: 

· This is an on-site, full-time position that is in a well-lighted, professional office environment.

Mental/Physical Requirements: 

· Involves sitting approximately 90 percent of the day, walking or standing the remainder.
