Job Title: Medication Aide

FLSA Status: Non-Exempt

Salary Range: TBD

Job Summary: Administers and documents medications to participants in the Adult Day Health Center. Reports directly to the ADHC Director.
Specifications

Education: High school diploma or equivalent. Currently listed as NA1 with NC Division of Facility Service with no substantiated findings of neglect, abuse, or misappropriation of resident’s property in a nursing home. Must have passed the Medication aide written exam and currently listed as a Medication Aide with NC Division of Health Service Regulation.

Experience: At least 1 year of working as a Medication Aide in Long Term Care/Assistant Living environment.

Number and Type of Employees Supervised (optional): None

Licensure, Registry or Certification Required: Valid NC driver's license and vehicle. Certificate of completion from a nursing tech/aide program and successful completion of a medication aide exam.

Special Training: Knowledge of physician orders and documenting orders. Able to communicate effectively both verbally and written form and document in the Electronic Medical Record, and Medication Administration Record. Knowledge of and care of adults with chronic health conditions. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by PACE of the Southern Piedmont, Inc. and approved by CMS before working independently.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact

Ages of Patients Rendered Care:

Neonate/Infant Early Childhood Adolescent Adult Geriatric All Age Groups

Key Responsibilities: (*denotes an age-related skill or task)

· Provides routine, and as needed medications that are given: by mouth, subcutaneous (insulin injections) aerosol treatments, topicals, nose, ears and eye medications and any other medications allowed per certification to participants while in the day center and ordered by the physician.

· Assists on outings/field trips as needed to give medications as ordered by physician, as needed.

· Obtains vital signs and weights on participants as ordered by the physician. Is able to change oxygen tanks as needed and document.

· Reports any changes in medical or physical status to Wellness Nurse or team member.

· Performs procedures and treatments within their scope of practice under supervision of Wellness Nurse.

· Attends staff meetings and is part of Quality Projects as applicable.

· Monitors and documents refrigerator temperatures and notifies Clinic Manager of temperatures that are out of range.

· Administers medication according to the 6 rights for med pass. Runs daily reports to monitor medications ordered. Documents services provided and observations on appropriate forms.

· Must be able to read insulin orders, draw insulin correctly and administer subcutaneous insulin injections. Correctly reads glucometer and documents blood glucose in the MAR

· Administers narcotic administration, counts and compares pills and signs narcotic log to keep correct medication count

· Checks participant blood glucose by correctly obtaining blood sample and is able to correctly clean glucometer and various equipment per protocol.

· Completes competencies in regard to giving/documenting medications and Infection Control annually and as needed.

· Supports the POSP Interdisciplinary Teams and promotes unity amongst team members’ while interacting with the team, other coworkers, and/or participants. Supports team members and functions as a team player while providing that support and adheres to the “do no harm” statement. Accepts teammate’s professional and cultural differences as they relate to participant care and supports team’s decisions while promoting a united team for the participants’ continuity of care and for staff development.

· Supports POSP mission by enhancing mobility, independence, quality of life, and quality of end of life for the participants
