Job Description - Post-Acute Admissions Facilitator

Job Description:

Job Summary


Responsible for supporting the discharge planning process by assisting with the coordination of patient transitions to post-acute and lower levels of care. Ensures timely completion of precertification and authorization activities to facilitate smooth patient transfers and reduce delays. Collaborates with case management, social work, and clinical teams to maintain accurate documentation and communication with post-acute providers. Applies basic knowledge of administrative and healthcare processes to support efficient patient throughput and continuity of care.




Essential Functions


• Initiates and completes precertification and authorization requests for post-acute care services in accordance with payer requirements.


• Communicates with insurance companies, post-acute facilities, and internal departments to coordinate discharge arrangements.


• Assists case managers and social workers in facilitating transitions to lower levels of care, ensuring all required documentation is accurate and complete.


• Tracks and updates patient authorization status in electronic health record systems to support timely discharge.


• Verifies insurance eligibility, benefits, and coverage for post-acute services and communicates findings to appropriate team members.


• Maintains organized records of authorizations, correspondence, and approvals for compliance and reporting purposes.


• Responds promptly to inquiries from patients, families, and care teams regarding post-acute placement and insurance processes.


• Follows established protocols to escalate delays, denials, or complex cases to supervisory staff for resolution.


• Adheres to confidentiality, quality, and compliance standards in handling patient and payer information.


• Performs other duties as assigned.




Education Requirement


High school diploma or equivalent experience

0-2 years of experience in a healthcare, clinical setting, or a relevant area




An equivalent combination of education and experience may be considered. All experience must be paid and in the same related field. Part-time and PRN experience will be prorated based on hours worked per week. Volunteer work and internships for academic credit are not counted.

Physical Demands


This position requires intermittent sitting at a computer workstation for extended periods of time; performing tasks with repetitive motions (such as typing); intermittent standing or walking with objects weighing up to 10 pounds; occasionally standing or walking with objects weighing up to 25 pounds; and occasionally lifting, pushing, and/or pulling objects weighing up to 50 pounds. It also involves occasional exposure to risk of injury from moving, lifting, or positioning patients, equipment, or materials; occasional risk of injuries from use of equipment on the job; and occasional exposure to cuts/punctures and bloodborne or airborne pathogens.
