Job Description - Post-Release Services Case Manager (L-2) / Home Study Caseworker


Classification:                  Exempt
Status:                               Full Time
Reports to:                        PRS Case Manager Supervisor
Salary and Benefits:         $51,000 annually; benefit package includes vacation and sick time, paid holidays,
                                                    insurance, pension and educational assistance

Summary of Function:
The Case Manager provides culturally competent assessment and case management services to recently immigrated unaccompanied children and their sponsors in the United States. Responsibilities include conducting Home Studies to assess safety and suitability of placements and delivering Post Release Services that promote successful community integration and family reunification. The Case Manager supports KYUMH’s trauma-responsive model by offering comprehensive services that enhance communication, provide psychoeducation, and connect youth and families to formal and informal community resources. This position works in collaboration with the Office of Refugee Resettlement (ORR) and serves families across Kentucky and potentially contiguous states. This role requires flexibility to work outside standard business hours, including evenings and weekends, as the HS/PRS program operates on a 24/7 schedule. Team members coordinate schedules to meet client needs, with approval from the Program Director or PRS Case Management Supervisor.

Required Skills and Abilities

Education and Experience Requirements
  • Bachelor’s degree ideally behavioral sciences, human services fields
  • Fluency in Spanish and English  
  • Knowledge of local community social services and specialized experience working with immigrant populations
  • Knowledge of family preservation, kinship care, and general child welfare.
Additional Requirements
  • Valid driver’s license and acceptable driving record.
  • Current First Aid and CPR certification.
  • Successful completion of background checks, fingerprinting, and drug screening.
  • Compliance with ORR vaccination requirements.
  • Ability to lift up to 25 pounds and perform physical tasks as needed.
Essential Functions and Competencies
  • Home Study Services
    • Conduct home visits, interviews, and assessments to evaluate sponsor suitability and safety.
    • Review background checks and supporting documentation for all household members.
    • Educate sponsors on reunification and assist with accessing necessary supports.
    • Prepare and submit comprehensive written reports within required timelines.
  • Post Release Services
    • Conduct home visits and phone contacts to assess placement safety and connect youth to community resources.
    • Develop individualized care plans with measurable goals, incorporating youth and family input.
    • Provide crisis intervention and safety planning as needed.
    • Facilitate case management meetings and monitor progress toward goals.
    • Ensure adherence to performance metrics for contact requirements and documentation standards.
  • Case Management & Advocacy
    • Act as primary liaison for youth, sponsors, and external agencies.
    • Coordinate referrals for services addressing education, health, mental health, legal needs, and family stabilization.
    • Advocate for access to critical resources and services for youth and families.
    • Maintain effective working relationships with families, colleagues, and community partners.
  • Compliance & Documentation
    • Submit detailed documentation in compliance with KYUMH and ORR standards, including case notes, assessments, care plans, and reports.
    • Demonstrate knowledge of ORR policies, legal requirements, and community resources.
    • Participate in professional development and maintain trauma-informed practices.
  • Travel & Outreach
    • Travel within a 200-mile radius and beyond as required (including air travel)
    • Engage in community outreach to build culturally competent interventions and support networks
