The Practice Transformation Specialist is an expert in health information documentation, data integrity, and quality. This role will be housed in the Cleveland Medical Associates clinic, lending support for the overall ACO success by streamlining processes and improving value based initiatives at CMA. The PTS will ensure the practice’s success of value-based care models in order to reach the highest achievement possible. It is their job to meet the individual metrics of each program we participate in. The PTS is responsible for taking action on all quality attribution reports from scheduling to closing gaps in care and submitting data to the various payers. The PTS will assist providers and staff with development of the skills necessary to adapt to best practices related to quality care workflows. The PTS is responsible for the accountability of the local team to achieve measurable gains within the programs, as measured by internal and external metrics. The position is responsible for various EHR maintenance responsibilities including training new clinical providers and is a liaison between EHR and clinical staff to improve workflow efficiencies and quality measures. This role will be required to perform annual wellness exams.
Licensure: Current TN or Multi-State RN or LPN required (Certified Professional Coder, Certified Coding Specialist, Certified Medical Assistant) preferred, but not required.)
Education: Associate degree in Nursing from an accredited nursing program as evidenced by primary source of verification. BSN or MSN preferred
Experience: Minimum 3 years’ related healthcare experience, hospital owned preferred. Must have Project & Physician-Provider Practice Relationship Experience
An equivalent combination of education and experience, which provides proficiency in the areas of responsibility listed above, may be substituted for the above education and experience requirements. Basic understanding of physician quality measures and credentialing requirements. Basic understanding of health care administration, clinic philosophy, and policies and operating procedures. Basic understanding of governmental regulations and reimbursement requirements.
Successful experience in use of practice management and electronic medical record systems. Basic knowledge of managed care industry, understanding of EMR quality management data and /or physician practice management applications required. This should be combined with a succinct understanding of physician practice operations including the effect of charge capture and clinical documentation on quality improvement.
