JOB SUMMARY


The Primary Clinician/Outreach & Recruitment Coordinator is a split role on a multi-disciplinary team focused on individual and group therapeutic engagement for youth experiencing early psychosis and their families; you will provide outreach and presentations in community settings to educate about OnTrack’s services and offerings, as well as interface with referral sources and potential participants through the referral process including, screenings, evaluations, and clinical consultations to diagnosis and determine eligibility.


ESSENTIAL JOB FUNCTIONS: List all essential job duties. Primary Clinician Duties:


• Serves as the Primary Clinician and point-person for a subset of OnTrack participants and their family; provides therapy, social skills training, recovery coaching, substance use treatment, and behavioral activation; and psychoeducation
• Partners with participants in supporting and clarifying steps toward accomplishing goals related to their recovery vision
• Facilitates youth and family-focused psychoeducation groups regarding first episode psychosis (FEP) and other appropriate topical groups to provide social engagement and practical life skills
• Conducts clinical assessments for participants and families in order to determine level of social functioning, substance use, and trauma to provide responsive services, connection to appropriate resources, and inform best practices
• Engages in safety-planning and wellness management planning; develops/modifies treatment using a shared decision-making (SDM) framework
• Utilizes strength-based documentation, treatment planning, and trauma-informed care;
• Attends weekly multi-disciplinary team meetings to discuss participant progress as well as ongoing training and supervision;
• Rotates after hour emergency call responsibilities with other clinical team members.
• Provides services in the community (off-site) as needed, based on participant and family preferences.
• Work with participants and family to transition from the team to their services and supports and ensure stable links at discharge
Outreach & Recruitment Coordinator Duties:
• Partners with Team Lead in serving as the primary contact person for receiving referrals, conducting screenings and comprehensive video/in-person evaluations, and identifying appropriate follow-up including document review, collaborative clinical reviews, diagnosing, collateral interviews, etc. Establish collaborative relationships with community partners like school centers and inpatient hospital units who might refer or provide ongoing support to young people and families.
• Engages potential OnTrack participants, and community partners through educational presentations;
• Maintains database of referral sources with up-to-date contact information.
• Refers youth found ineligible for OnTrack to other appropriate community providers


ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES


• Committed to the active promotion of ICL values and goals.
• Applies skill in basic use of technology including computers, applications, video sessions, email, electronic health records, and assessment platforms.
• Knowledge of mental health symptoms, conditions, substance use, harm reduction, and other therapeutic modalities.
• Knowledge of peer support programs, treatment, rehabilitation, and community resources.
• Knowledge of techniques for identifying and preventing potentially harmful behavior, including crisis management techniques.
• Ability to develop, evaluate, implement and modify a clinical treatment intervention to meet the needs of individual clients.
• Ability to prepare accurate and timely reports.
• Excellent oral and written communication skills.
• Ability to effectively and efficiently respond to questions from persons served.
• Ability to facilitate a meeting.
• Ability to serve as a role model to clients including modeling appropriate interpersonal interactions, appearance, demeanor.
• Willingness to continually learn and apply knowledge and willingness to participate in in-service training and development activities.
• Ability to function as an effective team member, including performing share of work, cooperating with coworkers, and securing cooperation, and maintaining professional relationships and boundaries with co-workers, supervisors and residents.
• Ability to use sound judgment in identifying and solving problems and knowing when to seek assistance.
• Ability to learn, understand and comply with all regulations, policies and procedures.
• Ability to organize time effectively to accomplish all tasks in a timely way and meet deadlines without prompting.
• Ability to develop, evaluate, implement and modify a case management plan, meeting all deadlines and productivity standards.
• Ability to prepare accurate and timely documentation, reports and other written material as assigned.
• Ability to identify the nature of problems and to participate effectively in solving problems.


QUALIFICATIONS AND EXPERIENCE


• Master’s degree or Ph.D. in social work, psychology, mental health counseling
• NYS License Required, L.C.S.W., L.M.S.W., L.M.H.C.
• Experience working with adolescents and/or young adults who are high-risk and who are experiencing serious or severe mental health symptoms.
• Experience in community-based mental health services.
