Role Overview
We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton. Immediate need: ~300 eligible residents are ready to be seen across our Stockton assisted living communities, providing steady, predictable volume.

What You’ll Do

• Routine & urgent assessments; chronic disease management
• Post-discharge follow-ups, med reconciliation, labs/diagnostics
• Coordinate with facility staff, specialists, families/POAs
• Same-day documentation; CMS/regulatory compliance

Schedule & Setting
Part-time; weekday blocks preferred. Clustered Stockton routes to minimize travel.

Qualifications

• Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
• Geriatrics or facility-based experience preferred; EMR proficiency
  

Compensation (CA pay transparency)
Annual compensation range: $150,000–$200,000 (based on experience, schedule, and patient volume)
Additional earning opportunities available based on coverage and panel needs
W-2 with benefits available; mileage reimbursed per policy
Final terms outlined in the employment agreement
