The Primary Care Physician provides comprehensive medical care to eligible veterans in an outpatient setting. This role includes preventive care, chronic disease management, and coordination of care within a multidisciplinary team, following VA and national clinical standards.
Provide comprehensive primary care services (preventive, acute, chronic)
Diagnose and treat conditions such as:
Diabetes, hypertension, COPD, heart failure
Perform mental health screenings (PTSD, depression, substance use)
Manage patient panels and follow-up care
Conduct telehealth visits and virtual care
Lead Patient Aligned Care Team (PACT)
Coordinate with specialists, pharmacists, social workers, and community care
Manage referrals, consults, and transitions of care
Maintain accurate records in VA Electronic Health Record (EHR)
Complete documentation and coding after each encounter
Follow HIPAA, VA policies, and clinical guidelines
Attend required meetings and training
Participate in quality assurance and performance improvement
Report patient safety incidents within required timelines
Monday–Friday, 8:00 AM – 4:30 PM
No weekends or federal holidays
MD or DO degree
Board Certified or Board Eligible in Internal Medicine
Active, full and unrestricted medical license in any U.S. state
DEA certificate
National Provider Identifier (NPI)
Eligible for VA credentialing and privileging
Strong clinical competency in primary care
Ability to read, write, and speak English fluently
Experience with EHR systems
Ability to work in a team-based care model

Experience in VA or federal healthcare systems
ACLS/BLS certification
Experience with telehealth and veteran populations
