Position Summary

The Primary Care Physician provides comprehensive medical care to eligible veterans in an outpatient setting. This role includes preventive care, chronic disease management, and coordination of care within a multidisciplinary team, following VA and national clinical standards.

🔧 Key Responsibilities

Clinical Care

• Provide comprehensive primary care services (preventive, acute, chronic)

• Diagnose and treat conditions such as:

  • Diabetes, hypertension, COPD, heart failure

• Perform mental health screenings (PTSD, depression, substance use)

• Manage patient panels and follow-up care

• Conduct telehealth visits and virtual care

Care Coordination

• Lead Patient Aligned Care Team (PACT)

• Coordinate with specialists, pharmacists, social workers, and community care

• Manage referrals, consults, and transitions of care

Documentation & Compliance

• Maintain accurate records in VA Electronic Health Record (EHR)

• Complete documentation and coding after each encounter

• Follow HIPAA, VA policies, and clinical guidelines

Administrative Duties

• Attend required meetings and training

• Participate in quality assurance and performance improvement

• Report patient safety incidents within required timelines

Schedule

• Monday–Friday, 8:00 AM – 4:30 PM

• No weekends or federal holidays

✅ Minimum Requirements

Education & Certification

• MD or DO degree

• Board Certified or Board Eligible in Internal Medicine

Licensure

• Active, full and unrestricted medical license in any U.S. state

Credentials

• DEA certificate

• National Provider Identifier (NPI)

• Eligible for VA credentialing and privileging

Experience & Skills

• Strong clinical competency in primary care

• Ability to read, write, and speak English fluently

• Experience with EHR systems

• Ability to work in a team-based care model
   

  Preferred Qualifications

• Experience in VA or federal healthcare systems

• ACLS/BLS certification

• Experience with telehealth and veteran populations
