Primary Care Physician – Family Medicine or Internal Medicine | Outpatient Practice

Job Responsibilities

• Provide comprehensive primary care services to patients in an outpatient practice setting
• Diagnose and treat acute and chronic medical conditions for patients of all ages (Family Medicine) or adult patients (Internal Medicine)
• Perform routine health screenings, preventive care, and wellness examinations
• Develop and implement individualized treatment plans and coordinate patient care
• Maintain accurate and timely electronic medical records documentation
• Collaborate with multidisciplinary healthcare teams to ensure optimal patient outcomes
• Participate in light call phone coverage rotation shared with other health system practices
• Engage in quality improvement initiatives and evidence-based medicine practices

Job Requirements

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required
• Board certified or board eligible in Family Medicine or Internal Medicine
• Active or eligible for New York State medical license
• DEA certification or ability to obtain
• Strong clinical skills in outpatient primary care medicine
• Excellent communication and interpersonal skills
• Commitment to patient-centered, community-based healthcare
• Ability to work collaboratively in a physician-led organization
