What You'll Do
As a Primary Care Physician, you will:
- Provide comprehensive primary care
services for adult patients
- Deliver patient -centered care with
a focus on prevention, chronic disease management, and wellness
- Accurately document patient
encounters and support value -based care initiatives through quality coding
and documentation
- Collaborate with an interdisciplinary
team to provide coordinated, high -quality care
- Participate in population health
initiatives and identify opportunities to improve patient outcomes
- Build long -term relationships with
patients while delivering compassionate, evidence -based care
What We're Looking For
The ideal candidate will have:
- Board Certification in Family
Medicine or Internal Medicine
- Significant clinical experience in
a primary care setting
- A passion for patient -centered care
- Experience with—or enthusiasm for—a
value -based care model
- Strong electronic medical record
(EMR) documentation skills
- Excellent communication and
interpersonal abilities
- A collaborative, team -oriented
mindset
- Flexibility, humility, and a
willingness to embrace innovation and continuous improvement
What Success Looks Like
Successful providers will:
- Deliver exceptional clinical care
while earning the trust of patients and colleagues
- Effectively identify and document
risk -adjustment diagnoses
- Support patients through preventive
care and chronic disease management
- Help eligible patients enroll in
advanced care management programs
- Progress toward an average of
approximately two patient visits per hour while maintaining a high
standard of care

Compensation & Benefits
Compensation: $200,000–$225,000 annually,
depending on experience.
