HealthFinders Collaborative (HFC) is a growing community health organization providing primary medical, dental, and behavioral healthcare, advocacy, and wellness education for people who experience barriers to care. We are seeking a compassionate, skilled, and collaborative Primary Care Provider to join our team and provide comprehensive care to patients across the lifespan.
The Primary Care Provider delivers high-quality, patient-centered medical care to children, adolescents, adults, and older adults. The provider works closely with clinical leadership and an interdisciplinary team to ensure that care is accessible, culturally responsive, evidence-based, and consistent with HFC’s mission and clinical standards.
HFC’s team includes medical, dental, and behavioral health providers, community health workers, support staff, volunteers, students, and community partners. Our staff bring diverse cultures, languages, lived experiences, and professional backgrounds to their work. Many team members are also growing in their careers and developing new skills and responsibilities.
Community health work does not always follow a predictable path. Patient needs and clinic priorities can change quickly, and the right provider will bring creativity, flexibility, compassion, and sound clinical judgment to the work. Successful providers at HFC value the contributions of colleagues in every role, communicate clearly, and help the team find practical ways forward when circumstances are complex.
This position is available on either a part-time or full-time basis, as well as school-year calendar if desired. The specific schedule will be developed based on organizational needs, the provider’s availability, and the agreed-upon FTE.
The provider’s schedule may include time in HFC’s community clinics, school-based health settings, or a combination of locations. HFC will work with the selected candidate to establish a schedule that supports continuity of care, team collaboration, and reliable access for patients.
HFC’s starting compensation ranges reflect the different professional qualifications and scopes of practice that may meet the requirements of this position:
This position is available on either a part-time or full-time basis. Compensation will be prorated based on the agreed-upon FTE. Starting compensation within the applicable range will reflect relevant experience, qualifications, scope of practice, assigned responsibilities, and internal equity. School-based and community-clinic responsibilities may both be included within the provider’s regular schedule and compensation.
HFC offers a competitive benefits package for eligible employees, including medical and dental insurance, 403b match, PTO and paid holidays, continuing education/licensure support, and malpractice coverage. Benefit eligibility and employer contributions may vary based on the employee’s scheduled FTE.
