About the Role:
We are seeking a dedicated and compassionate Private Duty LPN to join our team at Abundant Care. As a Private Duty LPN, you will have the opportunity to provide personalized care to our patients in their homes, making a meaningful difference in their lives.
Responsibilities:
- Administer medication and provide nursing care to patients in their homes
- Monitor patient vital signs and report any changes to the supervising RN
- Assist with personal care activities, such as bathing, dressing, and grooming
- Document patient care and progress in accordance with company policies
- Communicate effectively with patients, families, and other members of the care team
Requirements:
- Valid LPN license in the state of Pennsylvania
- Previous experience in home care or a similar setting preferred
- Ability to provide compassionate and personalized care to patients
- Strong communication and documentation skills
- Ability to work independently and as part of a team
