Your Role

Deliver expert clinical care with compassion and discretion in private residences
Conduct assessments, develop and oversee care plans
Collaborate with a multidisciplinary team (RNs, CNAs, companions, housekeepers)
Provide medication management, wound care, an health monitoring, post op and chronic care needs
Serve as a trusted resource for clients and their families

Who We’re Looking For:

Active NYS LPN Certification in good standing
Minimum 2 years of clinical experience (private duty, home care, or acute care preferred)
Impeccable professionalism and communication skills
Commitment to confidentiality and white-glove service
References required
