Job Description - Private Duty Nurse (PDN)

For over 30 years, PASCO has been providing industry-leading care and advocacy programs for a broad range of clients with disabilities and their families. PASCO is currently seeking a compassionate and dedicated RN to provide personalized one-on-one care for a male client in his early 20's who lives in Westminster. Client requires respiratory and GB cares.  This position will be needed on a part or full-time basis with a schedule of any consecutive days M-F working a 10-hour (8pm-6am) OR 12 hour (8pm - 8am) shift. Client has x2 hypoallergenic dogs and selected candidate must be ok with this.


POSITION
Private Duty Nurse (PDN)

POSITION PURPOSE
Provide direct, comprehensive nursing services for a single male client on an hourly basis overnight.  Additionally, this role will provide client education, complete and submit all required documentation, and may supervise or direct other assigned care providers. 

KEY DUTIES & RESPONSIBILITIES

  • Deliver skilled nursing care according to physician orders and industry standards.

  • Conduct holistic client assessments and implement appropriate care procedures.

  • Support clients in achieving care goals and adapt plans as their needs change.

  • Build and maintain positive relationships with clients and their support systems.

  • Coordinate care with the RN Case Manager, family, physicians, and other staff.

  • Maintain accurate in-home charts, medication records, and required documentation.

  • Report lab results and health changes to the care team promptly.

  • Participate in case conferences and ensure alignment with individualized care plans.

  • Supervise and guide LPNs, CNAs, and other care providers, maintaining quality standards.

  • Educate clients and caregivers, offering referrals to community resources when needed.

  • Adhere to HIPAA, PASCO policies, and state and federal regulations.

  • Communicate clinical updates and attend meetings for ongoing professional development.

  • Ensure reliable transportation, a valid Colorado driver’s license, and active insurance.
