PRN LPN or RN

Number of hours you will work is completely open. You will give us a schedule a month in advance and we will schedule them as appropriate. You must live on the Peninsula to be considered for the role. The job is paid on a per visit basis and travel throughout our service area, from the tunnel to as far north as Gloucester/Yorktown/Williamsburg.

Job responsibilities can include but are not limited to:

• LPN: Assist the Registered Nurse in carrying out the plan of care.


• LPN: Assist the Registered Nurse in performing specialized procedures (as defined in the State Nurse Practice Act) as directed by the Registered Nurse under physician's orders.


• LPN: Assist with the preparation, implementation and continuing evaluation of the patient plan of care.


• LPN: Assisting the patient with activities of daily living, including the teaching of self-care techniques;


• LPN: Preparing equipment and supplies for treatment that requires adherence to sterile or aseptic techniques.


• Help reinforce teaching to the patient/family members regarding end of life care.
  


• You will apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient’s eligibility. In addition, you will develop & implement a plan of care to ensure quality and continuity of care and recommend revisions to the plan as necessary.


• Determine scope and frequency of services needed based on the acuity of your patient and their needs, and their family. Also, you will assess the ability of other members of the care team to meet our patient’s immediate needs upon admission and throughout care.

Qualifications:

• Graduation from an accredited LPN or RN program.


• Must be licensed in the State of Virginia as a Licensed Practical Nurse or a Registered Nurse


• At least two years of nursing experience. Hospice experience highly preferred.


• Must be a licensed driver with an insured automobile in good working order.


• The ability to communicate well, both verbally and in writing, in English.
